Care needs better explanation

I was admitted into the hospital due to my mental health being at crisis point. Initially, I found that the care I received at the hospital was ok. However, as I got closer to being discharged, I didn't feel as ready as I wanted to be and when I asked questions and what was going to happen I didn't always feel I got a clear answer. When I was discharged, a lot of information that I felt should have been passed on to the community teams wasn't and so I had to fill in gaps so staff understood me. Once they did, the care was then good, and they helped me to make choices and decisions about my care and the future.
