Poor Care
My mum went for short term respite care from hospital to recover from an operation. Mum was isolated in her room for 14 days before an ambulance was called on day 14.

Mum was critically unwell and died a few weeks later. Care plan never completed with family, errors in risk assessments, cut and paste from other residents, poor diabetes care, poor dementia care and poor manager in place who didn't check her team leader and carers were doing their job.
