Care provider information confusing

When my husband was discharged from hospital, he received his six weeks social care entitlement which worked well. However, when this came to an end, we were bombarded with information about over 100 care providers who would continue his care for him. We felt like things weren't really explained properly to us and the information we did receive was confusing and overwhelming. Eventually we got it sorted, but it meant there was a substantial gap in his care, which was challenging for us. Now he needs two carers a day to look after him. Lots of my generation will be needing services like this more and more, so it's vital that it becomes more joined up with the hospital.
