It is designed to be constructive and allow both sides to have a fair and equal say in the matter so please:

Dreadful administration practices

I asked a very simple question, this is what happened: It is taken from the Surgery's web site and you need to read it from bottom to top. I have not asked for any documentary evidence, nor do I need it. I need someone at the practice to tell me what I have! COPD or asthma. However, in order for us to process this we need to see a form of ID as we are releasing information from your records. Please provide photo ID, this could be passport or driving license or a copy of utility bill, please see link provided with this response. I have printed a copy of a spirometry from 8.4.16 and a chest clinic consultation from 11.12.17. I need an ok from a GP to release and hopefully this will answer your question. If the practice is already aware of this problem, please tell us what has changed. On several occasions I have been informed that I have COPD or perhaps asthma. I now have to make a specific health statement for travel insurance where I must declare pre-existing conditions. Can you please advise me as soon as possible because I am required to complete the booking.
