As a service provider, you can leave your response to the original review and the reviewer will be notified of this. Lost medical record.

I wanted to check that I was still listed as a patient at this surgery, in preparation for the Covid jab, having been with five G.P.'s over 72 years, and was told that there were no records of me ever attending this practice. In fact, I was told I did not exist. I admit I have not needed to see a G.P. since 1984, but I am still alive and kicking, and have lived at the same address for the past 44 years. The practice finally 'sorted out the problem' by adding my e-mail address to a total stranger's documentation, also a [person with the same name] but with different middle names. I daren't contact them again, in case they do something more dangerous, and detrimental to her health. I have never given permission for my documentation to be either transferred or destroyed.
