Limited mental health support

I had a GP referral rejected by the Julian hospital and I have seen the mental health practitioner at the surgery but you only get six 40-minute sessions and it doesn't feel like enough. My last one was cancelled so I had it over the phone but I prefer face-to-face as it helps with my isolation. I've got one more session and I've not been told what the plan is or what's available afterwards. The social prescriber has told me to speak to my GP again as I'm still struggling. I will do that as I've also got a physical issue now. Some of them tell you to book online, it depends who you get over the phone, but the form is a big questionnaire which feels complex and overwhelming.
